A vendor receives a 4 out of 5 on "technical capability" because "they seemed strong in the demo," without any standardized definition of what a 4 means versus a 3 or a 5. This subjectivity undermines the entire purpose of the structured approach.

To prevent this, define scoring rubrics for each criterion before evaluating any option. For example, if one criterion is "implementation timeline," the rubric might specify: 5 = delivery in under 60 days, 4 = delivery in 60-90 days, 3 = delivery in 90-120 days, 2 = delivery in 120-180 days, 1 = delivery in over 180 days. With this rubric in place, scoring becomes an exercise in data collection rather than opinion formation. Each score is tied to verifiable evidence, which makes the entire evaluation defensible.

A second pitfall is criterion proliferation. Teams sometimes add criteria until the matrix contains 15 or 20 factors, each with a tiny weight. At this granularity, the matrix stops distinguishing signal from noise -- a minor advantage on a low-weight criterion can tip the total score despite being strategically irrelevant. The best matrices use 5-8 criteria, each carrying enough weight to meaningfully influence the outcome. If a criterion does not carry at least 5% weight, it probably should not be a separate line item.

The third pitfall is treating the matrix output as the final answer rather than as an input to judgment. A weighted score is a structured way to aggregate information, not a mechanical decision-maker. If the matrix produces a result that contradicts strong intuition, that is a signal to examine the criteria, weights, or scores more carefully -- not to blindly follow the numbers or blindly override them. In these situations, the process of building the matrix -- defining criteria, debating weights, gathering evidence for scores -- is often as valuable as the numerical output.

The matrix is less useful for binary decisions (do we enter this market or not?), decisions with a single dominant criterion (we need the cheapest option, full stop), or decisions where qualitative judgment outweighs quantifiable factors (should we hire this executive?). For these situations, other frameworks serve better: inversion thinking for binary choices, first principles analysis for foundational strategic questions, or red team exercises for stress-testing a preferred option against adversarial critique.

The real power of the decision matrix is cultural, not just analytical. Organizations that routinely use structured evaluation methods develop a shared language for decision quality. When anyone in the organization can ask "what were the criteria?" and "how were the options scored?" -- and receive a clear answer -- the quality of decision-making improves system-wide.
